I recognise that the sessions provided by Sound of Soma may include sound/vibration therapy and also light therapy. These services are designed for relaxation and self-care and are not a substitute for medical attention, examination, diagnosis, or treatment. I affirm that I am in good physical health and do not have any medical conditions that would hinder my safe participation in these therapies. If necessary, I have sought and obtained clearance from a licensed physician. I understand that while rare, there may be risks associated with these therapies, including but not limited to emotional release, lightheadedness, fatigue, physical tingling, and change in body temperature. I assume full responsibility for any risks, injuries, or damages, known or unknown, which I might incur as a result of participating in these sessions. Sound of Soma does not provide medical services, diagnosis, treatment, or advice. The Provider and its staff are not medical professionals, and any suggestions or recommendations are not a substitute for professional medical advice. I hereby fully release and hold harmless Sound of Soma, from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, that may be sustained by me, or to any property belonging to me, while participating in such activities. I agree to indemnify and hold harmless Sound of Soma against any and all claims, suits, or actions of any kind whatsoever for liability, damages, compensation, or otherwise brought by me or anyone on my behalf, including attorney's fees and any related costs.
If I am pregnant or trying to conceive, I have gotten consent from my doctor and I will advise the practitioner so that sound/vibration therapy will not be used on my body.
I have read this waiver of liability, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing the agreement freely and voluntarily, and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law. In the event that I require medical care or treatment, I agree to be financially responsible for any costs incurred as a result of such treatment. I am aware and understand that I should carry my own health insurance. This waiver shall be governed by and construed in accordance with the laws of the jurisdiction in which Sound of Soma operates.

Cancellation Pollicy
Cancellations or rescheduling requests made within 48 hours of your appointment will incur a 50% fee of the total session cost. Cancellations or rescheduling requests made within 24 hours of your appointment will incur a 100% fee of the total session cost. Please understand that these policies are in place to ensure fairness and allow sufficient time to offer the spot to someone else. Thank you for your understanding and cooperation.
